This is a contract between Four Oaks Puppy Club 4268 IH 10 S. Service Rd., Columbus,

Four Oaks Puppy Club agrees to exercise due and reasonable care, and to keep the kennel
premises sanitary and properly enclosed.
The pet (s) are to be fed properly and regularly, and to be housed in clean, safe quarters.
All dogs and/or cats are boarded or are otherwise handled or cared for by Four Oaks Puppy Club
staff without liability on Four Oaks Puppy Club part for loss of damage from disease, theft, fire, death,
escape, injury or harm to persons, other dogs or property by said dog(s), or for other unavoidable
causes, due diligence and care having been exercised.
Owner agrees to pay to Four Oaks Puppy Club
$13.50 (thirteen dollars & fifty cents) for small (0-20 pounds) dogs,
$14.50 (fourteen dollars & fifty cents) for medium (21-45 pounds) dogs,
$15.50 (fifteen dollars & fifty cents) for large (46-70 pounds) dogs,
$16.50 (sixteen dollars & fifty cents) for extra large dogs over 70 pounds per day for boarding
services.
Each additional dog may be kept in the same run for an additional $8.50 (eight dollars & fifty cents)
per day.
$9.00 (nine dollars) for one cat;
$7.00 (seven dollars) for each additional cat in the same kennel per day for boarding service.
Extra service fee:
Extra Private Play Time $5.00 per 15 minutes ______
Exit Bath (bath, air dry, no maintenance grooming) $12.00 -$15.00 ________
Medications: $1.00 per day. _______
Custom Diets: $1.00 per day (you provide diet) ______
Owner understands and agrees that dog(s) or cat(s) picked up by owner after 1:00 P.M.
Monday – Friday or 11:00 AM on Saturday, Sunday and Holiday’s shall be charged for a full
day’s stay.
All charges incurred by Owner shall be payable upon pickup of pet(s).
Owner further agrees that the pet(s) shall not leave the Kennel until all charges are paid by
owner to Four Oaks Puppy Club.
Kennel shall have, and is hereby granted, a lien on the pet(s) for any and all unpaid charges
resulting from boarding pet(s) at Four Oaks Puppy Club.
If the pet(s) become ill or if the state of the animal’s health otherwise requires professional
attention, Four Oaks Puppy Club, in its sole discretion, may engage the services of a
veterinarian of its choosing, or administering medicine, or give other requisite attention to
the animal, and expenses thereof shall be paid by Owner.
It is understood by Four Oaks Puppy Club and Owner that all provisions of this Contract
shall be binding upon both parties thereunto for this visit and for all subsequent visits. This
Contract contains the entire agreement between the parties.
